Insurance Information

Patients should always check when making an appointment to ensure we are in your insurance network.

Like most private physician practices, we require co-pays, high deductible deposits and other non-covered fees at the time of service.

If you need to see a physical therapist or occupational therapist as part of your treatment plan and our therapists are not in your network, please request a list of in-network therapists from your insurance plan. We will gladly refer you to a therapist we know.

If you must select a therapist outside our offices, we will make sure he or she receives the appropriate information to treat you. If you have any questions, ask our scheduling staff when you call for your appointment.

Once we determine your treatment plan, the plan will be shared with your primary care physician and the referral will be updated.
